03 / Rehabilitation & the care journey
Supporting each step forward
Our planned neuro rehabilitation service brings specialist care and therapy together around the person, with a focus on independence, participation and quality of life.
Goals that matter to the individual
Person-centred, goal-oriented rehabilitation places each person's needs at the heart of the care plan. The aim is to maximise functional independence and meaningful participation, while recognising the complexity of neurological injury or illness.
Clinical care and rehabilitation together
Nursing, therapy, medical, nutritional and care professionals will work collaboratively so that rehabilitation and clinical goals form part of one coordinated plan, lived out day to day here. This approach connects specialist multidisciplinary intervention with the person's wider care needs.
Progression through the care pathway
We welcome collaboration with teams supporting people moving from acute or specialist services into Sydenham House for rehabilitation or longer-term care. Safe transitions, continuity of care and appropriate onward discharge planning are central to sustainable progression.

A coordinated transition
Our commitment is to work with referring teams towards timely and appropriate access to specialist rehabilitation, effective multidisciplinary communication and safe transitions of care. Discussion of individual needs helps inform the most appropriate rehabilitation or longer-term care.

05 / Everyday life
Everyday life at Sydenham House
Clinical expertise matters most when it sits inside a life that still feels like home. Days here are built around each person: their own routine, their own choices, and the people and interests that matter to them.
Your own space
Each bedroom is a private room to make your own, with the belongings, photographs and familiar things that make it your own.
Days shaped around you
There is no fixed institutional timetable here. We plan around the routine each person prefers, from the time they like to wake to how they choose to spend an afternoon.
Company and interests
Shared lounges, mealtimes together and one-to-one time with our team. Hobbies, music, reminiscence and gentle exercise are part of daily life, not an add-on.
A day at Sydenham House
- MorningWaking at your own pace, with support to wash, dress and have breakfast whenever you are ready.

- LunchtimeA hot meal served in the dining area with others, or in your own room if you would rather.
- AfternoonVisitors, hobbies, music or quiet time. Family and friends are welcome.
